School Law Immunization Clinics Being Offered

The Sheboygan County Division of Public Health is holding special immunization clinics this summer to assist Sheboygan County students entering 6th, 9th, and 12th grades this fall to meet the recently revised Wisconsin School Immunization Law requirements.

Appointments are available for children ages 10 - 18.

The revised Wisconsin Immunization Law requires that: 1 Students entering grades 6 and 12 show proof of having received two doses of

chickenpox vaccine or a reported history of having chicken pox disease. 2 Students entering grades 6, 9, and 12 show proof of having received a dose of

Tdap or tetanus containing booster within the past five years.

Appointments are required and can be made by calling the Sheboygan County Division of Public Health at 920-459-3030 or 1-800-596-1919 (Ext 3030). Clinics will be held at UW- Sheboygan, One University Drive (corner of Taylor Drive and Indiana Avenue behind Lutheran High School), on:
1 Thursday, August 20, 10:00 a.m. – 6:00 p.m.
2 Friday, August 21, 9:00 a.m. – 4:00 p.m.
3 Monday, August 24, 12:00 p.m. – 6:00 p.m.
4 Tuesday, August 25, 12:00 p.m. – 6:00 p.m.
There is a $5.00 administration fee per injection. No one will be turned away due to an inability to make a donation.
