Transitional Living Program taking applications

Sheboygan’s Salvation Army Transitional Living Program (TLP) will be taking applications for a 25- person waiting list of potentially eligible clients for the 2012/2013 grant year.

Eligible individuals must be single adult residents of Sheboygan County with an income at or below 30 percent of the Sheboygan County median income ($1,220 monthly gross income). Potential clients must have documented mental health or AODA issues and be homeless or at risk of becoming homeless.

Clients accepted into TLP will be given rental assistance for up to 12 months and will be assigned a case manager who conducts home visits, individual counseling and gives referrals to other community resources.

Potential clients may call or come to The Salvation Army during office hours Monday-Friday, 8:30 a.m.- noon and 1-4:30 p.m. Upon request, applications can be mailed by calling TLP Case Manager Abby Dahmer at (920) 458-3723.
