PHS fall sports practice dates announced

Plymouth High School is announcing practice starting dates and other information for the 2014 fall sports teams.

Any athlete planning on competing in a PHS fall sport needs the following items:

• 2. A signed athletic code (parent’s and athlete’s signatures required).

• 3. A signed WIAA High School Eligibility Information Bulletin (parent’s and athlete’s signatures required).

• 4. A concussion information form (parent’s and athlete’s signatures required).

When the items are completed, they should be submitted to the PHS office and a blue permitto practice card will be issued. Athletes will not be allowed to practice until the blue card is submitted to the head coach. Blue cards may be picked up, starting Aug. 1.

The three items to be completed may be obtained by going to and scrolling down to Registration>Plymouth High School Registration>Athletic Forms step #4>Athletic forms. If you don’t have access to a computer, the items may be picked up at the PHS office during normal hours: Monday through Friday, 7-a.m.-2:45 p.m.

An athletic code meeting for athletes going out for sports during the 2014-15 school year will be held Aug. 20 in the PHS auditorium. Freshman parents and first-time athletes are required to attend the meeting which will begin at approximately 8:15 p.m., following freshman orientation.
