Assistance offered for first-time or minor passport applicants

County Clerk Jon Dolson and his staff will help U.S. citizens apply for a passport book and/or passport card Saturday, Nov. 4 from 9 a.m. – 1 p.m., at the clerk’s office located at 508 New York Ave., Sheboygan, on a first come first served basis.

This event is for first time applicants or minor’s passports that can not be renewed. Passport renewal applications are not processed by the County Clerk’s office, and need to be submitted by the applicant directly to the National Passport Agency.

Saturday hours on Nov. 4 ensure most parents will not have to miss work to apply for their children’s U.S. passport. Parents are encouraged to take advantage of this opportunity.

To help move the application process along on Saturday, it is recommended that each applicant complete the DS-11 Application on-line at Travel.State. Gov. Once completed, print the application on two separate sheets of paper (not back-toback) and bring the application to the office, along with the following required documents: Certified Birth Certificate (not a photo copy), Driver’s License/ State ID, check or money order for each application, separate check or money order (or cash) per application processing fee, and passport photo (the staff will take passport photos for $10).

Passports for children under the age of 16 expire every five years, and to apply for a passport for a child under the age of 16 both parents need to appear in person, along with the child. In addition, if the parent’s last name differs from the child’s, proof of the legal name change is required.

For anyone unable to complete the forms on-line, forms will be available in the office.

Persons unable to visit the County Clerk’s office on Nov. 4, may stop in Monday – Friday between 8 a.m. and 4:30 p.m. to apply for a passport.
